Visceral fat, or deep-lying abdominal fat stores, are considered the most detrimental to health. Having large fat stores deep in the abdomen sets you up for insulin resistance, diabetes, and cardiovascular disease. The best way to reduce such fat stores is through exercise and diet. The question is, however, what is the most effective exercise to reduce this dangerous type of fat?
Researchers from Duke and East Carolina Universities devised a study to focus on this issue, reporting their findings at the 2003 American College of Sports Medicine (ACSM) annual meeting in San Francisco in late May. The study subjects consisted of 111 overweight men and women, age range, 40 to 65, randomly assigned to either a control group who did no exercise,or one of three exercise groups:

- 12 miles walking each week
- 12 miles jogging each week
- 20 miles jogging each week
The study lasted 8 months. Results showed that those in the inactive control group gained 1.1 kilograms of weight, increasing their visceral fat deposits by 10%. The walking group lost 1.3 kilograms of weight, gaining 3.3% visceral fat. The 12-mile jog group lost 1.1 kilograms of bodyweight, and reduced visceral fat by 5.9%. The 20-mile jog group lost 3.5 kilograms of bodyweight, and 10% visceral fat. Only those in the 20-mile jog group showed a loss of subcutaneous fat (7.6%), or fat just under the skin.
The results of this study show that for purposes of reducing visceral fat, the more exercise, the better. In addition, the level of exercise intensity also is important, as the 12-mile jog group showed superior results to the 12-mile walk group. Another notable point of this study is that visceral fat continued to increase in the sedentary group, showing that future serious health problems are inevitable in obese people who don't exercise.

Erectile Dysfunction and Cardiovascular Problems


For men, flagging potency can be a red flag that somethings not right in the cardiovascular system. And experts say men who rush to fix the problem with impotence drugs may be ignoring a bigger threat to their health.

"Erectile dysfunction [ED] is frequently a manifestation of underlying cardiovascular problems. It even precedes angina - it can be an early warning, in that respect," said Dr. Andrew McCullough, director of male sexual health, fertility and microsurgery at New York University School of Medicine in New York City.

The advent of Viagra, Cialis and Levitra has put the research spotlight back on ED, and a series of studies are suggesting that, for most men, the condition has physiological, not psychological, origins.

Early in 2004, researchers led by Dr. Alan Bank of the St. Paul Heart Clinic in Minnesota published a study showing that the arteries of men with ED expanded less efficiently than men without erectile difficulties - a possible sign of underlying heart disease.

McCullough likened ED to a common plumbing problem: "If you turn on your kitchen faucet and you don't get any flow, either the faucet's broken or the pipes are clogged."

For men with advanced heart disease, the clogged-pipe explanation probably holds true, since buildup of cholesterol in arteries can greatly reduce blood flow.

But many men with ED aren't anywhere near this stage of cardiovascular deterioration. For them, the faucet - the vessels inside the penis and the smooth muscle around them -- may be broken on a biochemical level.

"We're learning that erectile dysfunction is largely due to reduced nitric oxide in the endothelial, the inner lining of blood vessels," explained Dr. David Gutterman, professor of medicine at the Medical College of Wisconsin in Milwaukee.

As blood flow demands increase (as happens during an erection), the endothelial releases nitric oxide to help trigger blood vessel expansion. "Nitric oxide is very important for normal erectile function," Gutterman said. "It's also - and here's the link to heart disease - very important as an anti-atherosclerosis agent, resisting 'hardening of the arteries.' "

In that sense, ED may serve as an early signal that blood vessels elsewhere in the body aren't expanding as they should, either.

It's no coincidence, then, that drugs such as Viagra - which helps increase nitric oxide levels in the endothelial - were first studied as possible treatments for cardiac trouble.

"However, it was found that these drugs do this better in some organs rather than others," Gutterman said. Looking at the problem from the other side, Bank's team is currently researching whether or not the statin family of cholesterol-lowering drugs might help fight ED.

"We do know that stating improve the function of the enzyme, nitric oxide syntheses, that produces nitric oxide inside the blood vessels," Gutterman pointed out. "That's probably related to the very mechanism that helps get rid of ED."

In the meantime, men with erectile difficulties should stop and consider the possible underlying cause of that dysfunction, McCullough said.

"Initially, when Viagra was first approved, there were all kinds of direct-to-consumer ads associating ED with a physiologic process, to convince men that it wasn't just psychological, and that it might be linked to underlying cardiovascular disease," he said.

"But now, there's a minimalization of all that in these ads. So guys who have these problems just say 'OK, I'll go online and get some Viagra,' " McCullough said.

But that type of attitude has its dangers, he said.

"One of the most serious things, in my mind, is the guy who fails Viagra, Cialis, Levitra. Because to me, that suggests he's got more serious underlying disease than some guy for whom it works," he said.

McCullough's advice? "Go see your doctor."

Gutterman agreed. "Some folks with undiagnosed heart disease might not get chest pain first, but they'll get erectile dysfunction. It's just another way of picking up on early coronary disease."

Premature Ejaculation


International experts from ten countries have teamed up to develop the first ever evidence-based definition of lifelong premature ejaculation (PE) in the hope that it will aid future diagnosis, treatment and research.

The results of their in-depth study, conducted by the Standards Committee of the International Society for Sexual Medicine, have just been published online by two leading Wiley-Blackwell publications, the urology journal BJU International and The Journal of Sexual Medicine.

They developed their definition after lengthy critical evaluation of the evidence presented in more than 100 studies on PE published over the last 65 years. It was unanimously agreed by the experts that the definition of lifelong PE should be a combination of three key factors:

* "Ejaculation which always or nearly always occurs prior to or within about one minute of vaginal penetration."

* "The inability to delay ejaculation on all or nearly all vaginal penetrations."

* "Negative personal consequences such as distress, bother, frustration and/or the avoidance of sexual intimacy."

However, the team have concluded that at the moment there is insufficient evidence to develop an evidence-based definition of acquired PE, which develops at a stage in a man's life rather than being a life-long problem.

They have also stressed that more research is needed into the PE issues faced by homosexual men as there is also insufficient evidence to develop a definition for this patient group.

The multi-disciplinary team was made up of 21 experts from the USA, UK, Singapore, Argentina, France, Egypt, Denmark, The Netherlands, Australia and Germany.

It included urologists, psychologists, psychiatrists, a sexual health physician, a primary care physician, a neuro-urology researcher, a clinical pharmacologist, an endocrinologist and a radiation oncologist.

"PE has been a recognised condition for many years and the first topical anaesthetic cream to delay ejaculation was described as far back the 1930s" explains co-author Dr Chris G McMahon, Associate Professor at the University of Sydney and Director of the Australian Centre for Sexual Health in Sydney.

"The premise that PE is a psychosomatic disturbance was first suggested in 1943 and men with PE were recommended to try various treatments such as the stop-start method.

"Various drug treatments have been developed over the last 30 years and in the past 15 years an increasing number of well-controlled, evidence-based studies have demonstrated the effectiveness and safety of selective serotonin reuptake inhibitor drugs in delaying ejaculation.

"More recently clinicians and the pharmaceutical industry have been paying more attention to the psychosocial consequences of PE.

"But what has been lacking is a consensus on how lifelong PE should be defined. As a result everyone - from urologists and clinicians working in sexual medicine to the pharmaceutical industry - have been working to different parameters.

"That is why the International Society for Sexual Medicine felt it was vital to bring together international experts to look at the wealth of published data and formulate an evidence-based definition of lifelong PE."

Men with lifelong PE suffer from a cluster of core symptoms, including early ejaculation nearly every time they have intercourse. Approximately 90 per cent ejaculate within 30 to 60 seconds of vaginal penetration and the remaining 10 per cent within one to two minutes.

The definition developed by the Society does not cover men with acquired PE, which develops in men who have previously had normal ejaculatory experiences. This may be due to psychological or relationship problems, erectile dysfunction, an inflamed prostate gland or thyroid dysfunction.

Because the definition has not been put into practice yet, it is not possible to quantify retrospectively how many men fall into the lifelong PE category based on the new evidence-based definition. However, previous research has suggested that as many as 35 per cent men suffer from premature ejaculation of some kind, making it even more common than erectile dysfunction (ED). Unlike ED, which increases with age, PE affects men more or less equally across all age ranges.

"Numerous organisations have tried to define lifelong PE but this is the first evidence-based definition to be developed" stresses Dr McMahon. "It is a vital step forward for men with PE and the clinicians who treat them as it can be used to provide consistent diagnosis and treatment and enable the results of any clinical trials to be compared."

Your doctor will first try to find why you have not been able to get pregnant. He or she will do tests to see where the problem lies. Sometimes doctors do not find a problem with either the man or the woman and don't know why a woman cannot get pregnant.

Treatment for female infertility

Problems with ovulating. If your doctor finds that you have a problem with ovulating, he or she may first recommend that you try the medication clomiphene (Clomid, Serophene, Milophene). This medicine (which you take as a pill) stimulates your ovaries to release eggs, thereby improving your chances of getting pregnant. It is often tried first because it is considered safe and effective.

Clomiphene has more side effects than hormonal shots, such as Pregnyl and Profasi, but clomiphene costs less, has a lower risk of ovarian hyper stimulation syndrome, and is less likely to result in a pregnancy with more than one baby.

Unfortunately, clomiphene does not always work. Typically, hormone shots are the next medication tried. You and your partner can weigh the risks and benefits of proceeding to this next step. You start the first series of daily shots at the beginning of your menstrual cycle. You will probably have mild side effects, such as feeling sick to your stomach and bloating. Some women have more serious side effects due to multiple, large ovarian cysts (ovarian hyper stimulation syndrome). While clomiphene increases your chance of having twins or triplets (especially twins), women who take hormonal injections are even more likely to have twins, triplets, or more babies.

Unexplained infertility. If your doctor cannot find out why you and your partner have not been able to get pregnant, he or she may start out by giving you clomiphene. The steps for treating infertility are essentially the same as for women who have ovulation problems. The next step is to try hormone injections. However, at this step your doctor may recommend insemination, putting the sperm directly into the uterus, to improve your chances of getting pregnant. If these treatments don't work, your next step is deciding whether to have IVF (in vitro fertilization).

Blocked or damaged tubes. Your doctor may do tests to check your fallopian tubes. Blocked or damaged tubes can prevent the egg from being fertilized by the sperm. If the blockage of your tubes is slight, your doctor might recommend tubal surgery to try to correct the damage. In these cases, between 20% and 60% of women have successful pregnancies after the surgery, depending on what part of the tube was blocked.6 However, in many cases, doctors recommend skipping tubal surgery and having IVF for more severe blockages. IVF is also often recommended first for women over 34 (regardless of the type of blockage) because tubal surgery and natural conception may use up precious time if in vitro fertilization might be used later.

Endomorphisms. If you have mild to moderate endomorphisms that seems to be the main reason for your infertility, your doctor may use laparoscopic surgery to remove geometrical tissue growth. If surgery does not work, or if you have severe endometriosis, you will need to decide whether to try in vitro fertilization, commonly called IVF. However, understand that IVF doesn't work as well for women with endometriosis as with other causes of infertility.
